1. Data Controller

Your personal data is processed by Op. Dr. Mehmet Ataman (Tunalı Hilmi Cad. 110/15, Kavaklıdere, 06690 Çankaya/Ankara, Phone: 0312 428 37 37, E-mail: mehmet.ataman@gmail.com) acting as data controller, within the scope described below.

2. Data Processed

Identity: name, national ID number, date of birth
Contact: phone, address, e-mail
Health data (special category): complaints, history, examination findings, diagnoses, test results, treatments and operations, prescriptions
Financial: invoicing and payment details

3. Purposes

Provision of medical diagnosis, treatment and care
Appointment scheduling and patient communication
Keeping and retaining patient records as required by Turkish health legislation
Invoicing and fulfilment of financial obligations
Notifications to authorised institutions where legally required

4. Legal Basis

Your personal data is processed under Art. 5/2 of the KVKK (express provision in law; fulfilment of legal obligations), and your health data under Art. 6/3 — for the purposes of medical diagnosis, treatment and care, by persons under a duty of confidentiality.

5. Transfers

Your data may be transferred only where legislation requires (Ministry of Health systems, courts and authorised public bodies), to healthcare institutions and laboratories involved in your care, and to our accountant for financial obligations. It is never shared for commercial purposes or transferred abroad.

6. Retention

Patient records are retained for the statutory periods under Turkish health legislation (a minimum of 10 years for private practice records) and destroyed thereafter.

7. Your Rights (Art. 11 KVKK)

You may learn whether your data is processed, request information, learn the purpose, know third-party recipients, request correction or deletion, object to adverse automated outcomes, and claim damages for unlawful processing. Applications may be submitted in writing to the practice address or to mehmet.ataman@gmail.com; they are answered within 30 days at the latest.
